Dear Parents of Student-Athletes,

Researchers at the University of Minnesota are interested in looking at how COVID-19 affects high school athletes with long COVID symptoms and are asking for your help in this important research. We know from previous research that National Collegiate Athletic Association (NCAA) athletes who answered a similar survey had about 3 cases of long COVID-19 per 1,000 athletes. However, little research has been done on this topic with younger athletes.

On behalf of the team at the University of Minnesota, Department of Family Medicine and Community Health, we are sharing a voluntary survey (linked below) that will ask questions about your student’s experience with COVID-19, such as when they had the disease, when they returned to sport, what symptom(s) they had or still have, vaccination status and dates, and what sport(s) they play.

Your student’s participation in the survey is voluntary. Your student’s answers in the survey will be placed in a de-identified data base and will not be associated with their name or other identifiable information.

Thank you for your consideration in allowing your student to participate in this important research.
